Abstract
Background and significance: This quality improvement project implements a video component to the discharge education process for heart failure patients. Heart failure is a progressive multisystemic disease that initially affects the heart and then affects other organs, such as the kidneys and lungs. Over time, it can develop into multisystem organ failure (D’Amato et al., 2024).
Purpose: Centers for Medicare and Medicaid Services (CMS) rewards and reimburses hospitals based on their Hospital Consumer Assessment of Healthcare Providers and System Survey (HCAHPS) performance scores. Survey scores are higher in facilities with lower mortality than those with higher mortalities (NRC HCAHPS, 2018). This quality improvement initiative will analyze pre- and post-project survey scores during the implementation period compared to the HCAHPS survey responses.
Methods: This quality improvement initiative compared the HCAHPS survey scores from the pre- and post-project implementation. The data calculated and displayed on a bar graph that noted an improvement in both discharge education and care transitions. Care transitions scores increased from 43.9% to 55.9%. Discharged education scores increased from 71.4% to 81%.
Recommendations: Expand the data collection to evaluate HCAHPS scores over a three-month period to allow for further data collection. Expand to all heart failure patients throughout the hospital and the hospital network.
